Our regional health system spans five counties in southeastern Michigan. Our home care and hospice professionals travel to bring our special brand of personalized care to the patients. Hospice professionals provide care in several settings including in-home intermittent care, facility-based care (SNF or ECF), continuous home care or inpatient. Our service area includes Washtenaw, Wayne, Lenawee, Monroe, Jackson and Livingston County but we are committed to caring for our patients wherever they may live.
