After completing the forms to the best of your ability, please bring them to your first appointment in the clinic. Parents, please complete the Confidential Information Packet for Children and Adolescents for your child. All children 13 years of age and older also need to complete the Confidential Information Packet for Adults.

Please note that there are additional forms other than the forms listed above that will need to be completed prior to the appointment. The above forms are provided for you to complete at home at your convenience. They are also available at check-in, however filling out of the admission forms above generally takes 45 minutes, so if you do choose to complete the above forms at the clinic please arrive early enough to complete all forms before your scheduled appointment.
